Getting Tested
In order for students to be tested, parents must fill out this online waiver. Paper waivers will not be accepted. You may scan the QR code in the images attached, or click on this link below to complete the online waiver for Poplar Health. Please enter both a valid email address and phone number in order to be tested:

Getting Results
Poplar Health will contact the parent directly in the case of a positive Covid result. Please notify the front office immediately if a positive result is reported to you and keep your student at home and seek medical attention from a provider if need be.
